Hi, I received a diagnosis for autism a few weeks ago. I also receive Universal Credit, previously ESA.

I feel the DWP are pushing me a little too hard to be honest. I have monthly meetings with them. At the last, they sent me a docuement called a Health Adjustment Passport saying it was to help people with disabilities avoid discrimination and find work. But it's just a massive list of quesitons, like the Work Capability Assessment. I don't really understand what I'm meant to do with this, or how it works. I can't see many employers being willing to read through it.

Does anyone know a bit more about this?
 
Its main purpose is to help you figure out and record what reasonable adjustments (e.g., flexible hours, adapted equipment, remote working, altered tasks) you need to do a job safely and comfortably

It can fast track access to work application if you are offered work, to help with adjustments you might need

It avoids repeating your story, because you fill it in once so you don't have to keep re-explaining complex medical conditions or personal details every time your Work Coach changes or you start a new role

it's a voluntary document meant to assist you, not a mandatory benefit requirement, you can actually ask your work coach to help guide you to fill it in, as you might not know what kind of adjustments you need or are reasonable

Focus on barriers, not medical history

You don't need to explain your whole medical diagnosis to your Work Coach.

Keep the focus strictly on practical impacts, for example: Struggles with noisy environments, rather than deep medical details.
 
But the questions don't seem to align with that.

From what I know of Access to Work it's, very basically, just someone you can talk to if you're having struggles at work. They don't interact with your employer, so I'm not entirely sure how they can help.

I'm very confused by this. It was sold to me as a general purpose thing i can take to any job, but the questions seem to suggest otherwise. For example, it asks me what job i'd like to do (make music at home please), but that's not going to sit well with an employer as they will just think i'm unreliable or ultimately disinterested, for example. I can't imagine any empliyer is going to want to read through this entire form.

I think it's more of a guide for you to use with your work coach and in interviews you can use it to remind you of any reasonable adjustments you might need, it's supposed to help you be able to advocate for yourself
