Diagnoses support?

Hello, I am currently in the process of getting a diagnoses for add/hd and strongly suspect myself to be on the autistic spectrum too.
I am 53 and have 2 children, one has add and autism, the other has dyspraxia.
I have been given a 10 page form to fill in before I can see a doctor re assessment.
I am wondering how other people have found the process of diagnoses in adult women in the uk? Is there any support I can look for to help me with the form? Seems completely unreasonable to expect someone with add to fill in a form on a deadline..
Any suggestions or just stories of personal experiences would be greatly appreciated!

hi my name is Morgan and I got diagnosed in July for ASC and am undergoing ADHD dx
I'm non-binary so not sure about the specifics of your experiences of women as dx but I would say breaking down the form into doing say 2 pages a day really helped me as well as filling out with the person who was doing my Informant report.
obviously, you're not meant to speak to each other but just sitting opposite my mum as she did hers really helped because it was somebody to keep me accountable.
I also found breaking it down into like sections was really helpful so I could narrow in on one specific part of my experience without having to think about my life as a whole which was quite overwhelming.
I filled mine in virtually so I typed and use the dictation software (speech to text) which is on almost every laptop and phone so that can also help if you just wanted to explain how your feeling but find writing or typing difficult. I know with my ADHD I can sometimes get overwhelmed because I write much much slower than I think.
in most of the referral forms remember you're only doing a bullet point or two per question because the Professionals you are speaking to often don't have time to read through a huge essay so just keep it concise and to the point and answer the question as directly as possible.
if you don't know the answer to a question you can just write that in the box I did that several times and you can also ask for clarification from the people around you or email in your question to whoever is doing your assessment.
finally you can also email whoever is doing your assessment and just explain your problems because I doubt you are the first and see if they have any guidance.
